The Transitions Action Coalition (Transitions) is a community coalition comprised of members from various youth-serving agencies and community interests in Lanark County.  Transitions was formed to identify and address the housing needs of transition-age youth (16-24) in Lanark County. Transitions hopes that through partnerships and collaborative efforts, this gap can be bridged so that these youth can become self-sufficient and live independently.
